Filing a Claim with the City

If you wish to file a claim with the City, please:

1. download a claim form, or
2. pick up a form at Chino City Hall, 13220 Central Avenue, Chino, Monday through Friday, between 7:30 a.m. and 5:30 p.m.

Once completed, the Claim form may be mailed or delivered to:

Please be sure to include any quotes, receipts, photos, or other documents to support your claim.
